About This Event

This course, sponsored by NCMIC, addresses the complexities of Evaluation and Management (E/M) coding for chiropractors. Dr. Gwilliam simplifies the 2021"“2024 E/M changes, highlighting which rules are applicable in a chiropractic setting and providing practical examples and note verbiage to ensure accurate selection and documentation of service levels. Attendees will learn to interpret E/M guidelines, differentiate between MDM-based and time-based code selection, and construct compliant documentation that supports the chosen E/M level and withstands payer review. The second hour of this program tackles the common challenge of performing and documenting re-exams on the same day as a CMT. Dr. Gwilliam guides participants on identifying clinically justified re-evaluations, documenting them as "significant, separately identifiable" services, and correctly applying modifiers and diagnosis pointers to minimize denials while maintaining ethical compliance. The session will enable attendees to apply payer and coding guidance, including the 25 modifier and NCCI edits, to create accurate and defensible re-exam documentation, distinguishing between routine care and medically necessary re-evaluations.

Learning Objectives

  • Interpret and apply the 2021-2024 E/M coding guidelines specific to chiropractic practice, including MDM-based and time-based code selection methodologies

  • Apply the 25 modifier and NCCI edits correctly to same-day re-exam and CMT billing scenarios to minimize denials
  • Construct "significant, separately identifiable" re-exam documentation that demonstrates medical necessity while maintaining ethical coding compliance
  • Utilize diagnosis pointers and modifier placement strategies to defend re-exam claims during payer review
